Vitamin D influences how the immune system works, but as a "cold remedy" it works modestly and not for everyone. Regular intake slightly lowers the risk of acute respiratory infections — most noticeably in those who were deficient to begin with. If your vitamin D level is normal, extra doses "for immunity" change almost nothing. Here is what the research actually shows and who a supplement genuinely makes sense for.
What vitamin D does in immunity
Vitamin D is not only about bones. Its receptors sit on immune cells, and it helps regulate their work: supporting the antimicrobial defence of mucous membranes and dampening excess inflammation. This is a biologically plausible basis to expect an effect against respiratory infections — and it is exactly what studies tested.
It is important not to conflate two different claims. "The immune system needs vitamin D" is true: with marked deficiency, much suffers, including defence against infection. "The more vitamin D, the stronger the immunity" is false: above the normal range there is no added benefit, and very high doses are harmful.
What the studies showed
The key source is a meta-analysis of individual participant data (Martineau, BMJ 2017) and its update (Jolliffe, 2021). The overall conclusion is robust: regular vitamin D lowers the risk of at least one acute respiratory infection, but the effect is modest and depends on conditions.

Two practical conclusions follow. First, the benefit comes from correcting a deficiency, not from pushing the level above normal. Second, the regimen matters: steady daily intake is more effective than rare large doses.

Who genuinely needs a supplement
The point of a supplement depends directly on whether you are deficient. In the groups at risk of a low 25(OH)D level:
- people at northern latitudes in the cold season;
- those with little sun exposure;
- people with darker skin, older adults, those with obesity.
For them, correction makes sense for both immunity and bones. But for a healthy person with a normal level, taking vitamin D "for immunity" in the hope of not falling ill is overestimating the effect.
What about COVID-19
There was great hype around vitamin D and COVID, but it ran ahead of the evidence. Yes, deficiency is associated with more severe disease — but this largely reflects overall poor health (obesity, chronic conditions, older age) rather than a direct cause. Randomised treatment trials gave conflicting, mostly modest results. Keeping vitamin D in the normal range is sensible, but it is not a medicine for infections.
What this means in practice
The strategy is simple: keep vitamin D in the normal range rather than chasing an "immune" maximum. In the cold season and in at-risk groups, correct a deficiency with an ordinary daily dose, ideally guided by a blood test. And remember that for protection against colds, sleep, movement and diet do just as much — often more.
